It took me 2 years, 3 Clomid cycles, 5 IUI cycles and 1 IVF to finally become pregnant with twins. They were delivered at 35
weeks in an emergency C-Section, because my son's placenta had abrupted. I lost an incredible amount of blood from not
only the abruption, but once they had delivered the twins, my uterine artery started bleeding as well. They had to give me 4
units of blood, and I was on the operating table for over 2 hours. What are my chances of having another serious abruption
again if I get pregnant with a singleton?

Answer by Dr. Vanessa Garner (doctor) on Thu 08, Oct 2009 06:04pm:
Hello! Welcome to the site and thank you for posting. I understand your worries regarding this condition.
An abruptio placenta is a case wherein there is separation of the placenta from the uterus prior to delivery
of the fetus. In your case, having twin gestation placed you at a higher risk for placental abruption. Other
risk factors include hypertension, cigarette smoking, cocaine use, increased maternal age, abdominal trauma
during pregnancy, and also chorioamnionitis. It is estimated that there is around 5% to 10% chance of
recurrence in your subsequent pregnancy. Though there are some reports that say the chances even rise to 17%.
This rate increases to greater than 20% if you have two consecutive pregnancies complicated by the condition.
If you are a smoker, quitting can greatly decrease the chance of abruptio placenta from happening again. We
hope this answers your question. If you need clarifications, don’t hesitate to post your queries again. If

Clarification by emk808 on Fri 09, Oct 2009 10:02am:
Thanks! My only risk factor for abruptio placentae was having twins--my BP was good, I don't smoke, etc,
and I didn't have any trauma. That's why I was wondering, because I don't expect to ever have
twins again. :-) The twins are 6 1/2 months old, so it makes me happy to know that even if I were to get
pregnant by accident tomorrow, that I'm basically healed. It also didn't happen until I was 35
weeks pregnant and had a fundal height over 50cm, and I definitely will never get that big again! Does this
make my chances of recurrence go down?

Hi, Thank you for further using our services. Yes, I agree with your opinion that the chances of recurrence of
abruption of placenta is very less ( as other risk factors, apart from twin pregnancy, are ruled out). But, it
will be too enthusiastic to say that you are 100% safe. Also, getting pregnant with bad gynecologic history
and two kids of 6 1/2 months will be overexpectation by you. We feel happy to hear you relieved. Take care.

Response by Dr. Srivastava on Fri 09, Oct 2009 12:20am:
Hi, My point of view is also in complete agreement with Dr Vanessa. “Abruptio placentae” is premature
disruption of placental attachment from uterus lining before the birth of child. Firstly, I would say that it
is not “a rule of thumb” that if first pregnancy had a placental disruption in your case then there is 100%
chance of history repeating itself. Though the risk is higher then females without such history of problem
(values are given by Dr Vanessa). I feel the probable cause of ‘Abruptio placentae’ in your case is twin
pregnancy. Though, other possible causes are trauma, maternal hypertension etc. So, if you will have a
singleton pregnancy in future (esp. after a good healing period of atleast 6 months or more) then you may not
have to face such consequences. But, still you need to keep your fingers crossed and take proper care and
required rest in the future pregnancy (esp. keeping a check on BP). Any ways! Congratulations! Enjoy your
motherhood for now. Hope this satisfies your quest and in case you have future queries then do write to us.
Take care and regards
