I have polycystic ovarian syndrome and have been on metformin for about a year. I take 1000 MG in the morning and 500 MG in the evening with a prenatal vitamin. I have had my period like clockwork on the 18th of every month for the past 6 months. But this past month I have not had my period yet. I have taken two pregnancy tests and both came out negative. My husband and I are trying to have a baby and my not having my period on time every month really scares me. What could cause this? Could I be pregnant?

Hi! I can understand your concern. Since you have missed your periods and the urine test is negative, take a blood pregnancy test for confirmation. With the treatment of PCOS, at times ectopics do happen and pregnancy test remains negative at times. Hence if blood test is negative, a transvaginal ultrasound should be done if the gynecologist who assesses your case feels the need. There are many reasons to miss periods. Stress whether emotional or physical can delay the period. Hypothyroidism, pituitary tumors secreting prolactin, nemia and poor diet and unexplained or sudden weight gain or weight loss are the other factors to be looked into. If you and your partner have been trying for kids for a year and you have not had kids earlier and this time the missed period is not due to pregnancy, then you should consult an infertility specialist. There are various reasons for this like ovum not being formed, hormone imbalance probably due to PCOS, hypothyroidism, local infection, blocked tubes etc. A vaginal examination, PAP smear, STD panel, a complete hormone panel (thyroid, estrogen, progesterone, FSH, LH and prolactin), CT scan or ultrasound, hysterosalpingogram, semen analysis, etc will need to be done. It is difficult to really comment on your possibility of pregnancy right now. However I sincerely hope this post helps you understand the course of action to be taken. Hi, As Dr Kokil has said it is difficult to say whether you are pregnant right now . It needs confirmation by doing serum beta HCG levels & a transvaginal ultrasound . As your pregnancy tests have come negative twice , the chances of pregnancy are less. In case there is no pregnancy , you will get withdrawal bleeding by taking progestins.You are having PCOS, inspite of which you have been having regular periods. Irregular &scanty periods along with excessive weight gain, excessive hair growth are the symptoms associated with PCOS. You may do fasting insulin & blood sugar levels to find the extent of the hyperinsulinemia that occurs in PCOS. You may continue with the metformin. In the meantime other investigations need to be done to find out as to why you are not conceiving . Hypothyroidism & hyperprolactinemia may lead to anovulation. Your husband’s semen analysis needs to be done . Also we need to see if the fallopian tubes are open either by doing hystero-salpingography or laparoscopy. All things being normal you can easily conceive by induction of ovulation & IUI – intra uterine insemination. You may discuss the above mentioned with your gynaecologist.
